Description: The CBCS team supports individuals aged 3-99 diagnosed with an intellectual and/or developmental disability who have experienced a crisis episode or who are at risk of experiencing a crisis episode. CBCS Coordinators support individuals with IDD to remain in the community by providing long-term crisis prevention and management services. CBCS works systemically with all members of the client system to assist in the development of crisis prevention and management strategies.
As a member of the CBCS Team, you will work with individuals and their systems of support to identify what puts individuals at risk of crisis. You will work closely with the IDD Division staff as well with staff throughout the agency to ensure clients are receiving the necessary services and supports. Many of the individuals you will serve will have co-occurring diagnoses such as severe mental illnesses and chronic health conditions.
Responsibilities and Duties
- Engage in collaborative crisis prevention and management planning with the individual, their system of support, and other service providers
- Complete narrative assessments and develop individualized crisis plans
- Maintain a caseload of CBCS clients
- Provide consultation and/or training to caregivers, service providers, and law enforcement on the unique needs of individuals diagnosed with IDD
- Maintain documentation of all client contacts within required timelines and in compliance with program requirements
- Participate in inter-agency collaborations to problem solve client needs and barriers to crisis prevention
- Provide support to individuals in accessing appropriate services through communication within Integral Care, community partners, service providers and other relevant agencies
- Travel locally, daily, for client visits
- Utilize electronic health records, Microsoft Word/Outlook and other applications

Education: Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Human/Behavioral/Social Services or related field is required. Master's degree is preferred.
Experience: At least one year providing services to individuals diagnosed with intellectual and developmental disabilities is required.
License: Center certification to perform service coordination is required. (Training will be provided during the onboarding process to obtain this certification.) Current and valid Texas driver's license and a clean driving record required.
Hours: Monday -Friday, 8am-5pm with some with some flexibility required.
Salary: Starting Qualified Mental Health Professional (QMHP) salary is at $38,479-$40,739 based on education, but internal applicants are subject to Integral Care's QMHP career ladder.
